If you’re having trouble accessing PATIENT PORTAL, then follow these guidelines:
- Return to original email you received with the Patient Portal link and review the instructions for how to set up your Patient Portal account.
- Remember to use the same email address from the email you received as the one you enter on the Patient Portal log-in.
- If you still can’t enter the portal, call us at (561) 513-9313 to request a link to forms that can be completed electronically OUTSIDE of the Patient Portal.
- You may also request these forms by REPLYING to the original email, or simply email frontdesk[@]emdecares.com.
- As a last resort, you may fill out forms manually at our office, but please come in at least 20 minutes in advance of your appointment time.
